This is a form to help Dr Mark get stuff done for you. For new patients and general enquiries, use our Contact form.

This is where you can place requests for repeat prescriptions, letters and certificates in here. As well, the COVID-19 and TeleHealth changes (with much higher demands on Mosman Integrative Medicine) may mean that pathology requests, letters, notes and other documents promised By Dr Mark may not have been sent. You can let us know what those documents are so Dr Mark can complete and send them.

Please be specific. With prescription requests, the name of the medication, the strength and the number in the pack are usually on the label of the current medications. If you want them faxed to your pharmacy or a compounding pharmacy, please include the pharmacy name and fax number. Please put those details in.

For other forms, please be as specific as possible, and let us know any deadlines. Amarina will get back to you by email or phone if clarification is needed, or when the documents are ready.
